This outcome measures report describes the changes in outcome measure results relative to the baseline, allowing us to track our progress towards achieving the outcomes over time in collaboration with others. The baseline is shown in brackets and further defined on the webpage.

There are 25 outcome measures and at the end of 2024/25: six targets were met, five targets had no change, five targets were not met, six measures have a baseline only, (which will be reported against in future), and three measures are in development, (i.e. there is no baseline or result yet and we are still working on the best way to report.)
